Health Canada is pleased to announce the release of the final Guidance Document for Industry -- Issuance of Health Professional Communications and Public Communications by Market Authorization Holders MAH ; . Health Professional Communications HPCs ; and Public Communications PCs ; are one of the principal means used by industry to communicate new information about safety and therapeutic effectiveness of marketed health products to health care professionals and the public in a timely manner. This guidance document clarifies the roles and responsibilities, the issuance process, the content and the timelines to assist MAHs in developing and disseminating HPCs and their accompanying PCs. In August 2004, a draft version of this document was released for stakeholder consultation. A total of 183 comments were submitted by stakeholders, including pharmaceutical companies, pharmaceutical associations and academic centres. The comments were related to technical issues, clarifications and other issues. The final version of the guidance document has been updated in light of these comments. The Marketed Health Products Directorate would like to thank all those who participated in the consultation. We report that cells with a number of properties consistent with those of dopaminergic neurons can be efficiently generated from a hESC line, BG01, in vitro by coculture with PA6 cells. Case report A 69-year-old retired clergyman, who had undergone a partial gastrectomy for a duodenal ulcer in the 1960s, presented in late 2004 to his GP with fatigue. He was found to have a normocytic anaemia Hb 7.7g dl, MCV 88.5fl ; and leukopenia white cell count 1.1 * 109 l ; in the presence of a normal platelet count and normal B12, folate and ferritin levels. He was referred to the haematologists at his local hospital where a bone marrow biopsy showed dyserythropoiesis with vacuolated erythroid precursors, left shift of granulopoiesis, and normal megakaryocytes. Perl's stain demonstrated normal iron stores and ring sideroblasts. A diagnosis of myelodysplastic syndrome was made. He was managed supportively over the next 18 months with recurrent red cell transfusions, as required. His white cell count remained low, with neutrophils never exceeding 0.9 * 109 l Figure 1 ; . He was first seen by neurology 12 months after initial assessment having complained of ascending numbness. Examination revealed a mild spastic paraparesis with brisk lower limb reflexes, bilaterally upgoing plantars and a soft sensory level at T11. He was fully ambulant. A compressive spinal cord syndrome was initially suspected. MRI of his spine was unremarkable and, apart from his known haematological abnormalities, all investigations were normal including repeated B12 and folate levels, autoantibody screen, antineutrophil cytoplasmic.

Pigmented nevi are rare causes of focal oral pigmentation. They present as either brown or blue lesions. Histologically, nevi are composed of an accumulation of nevus cells in the basal epithelial layers, the connective tissue or both. As such, they are classified as junctional, intradermal or intramucosal, and compound nevi. Junctional nevi are flat and dark brown in colour because the nevus cells proliferate at the tips of the rete pegs close to the surface. Intramucosal and compound nevi are typically light brown, dome-shaped lesions. Blue nevi are characterized by proliferation of dermal melanocytes within the deep connective tissue at some distance from the surface epithelium, which accounts for the blue colour. The intramucosal nevus is the most common type and is seen most frequently on the buccal mucosa.28 The blue nevus is the second most common type, occurring most commonly in the palate.28 The mean age at excision is 35 years. It may be difficult to differentiate clinically between a nevus and an early lesion of mucosal melanoma, especially in the palate, the most common site for both lesions. Although transformation of oral pigmented nevi to melanoma has not been well documented, it is believed that nevi may represent precursor lesions to oral mucosal melanoma.29 It is therefore recommended that these lesions be excised and submitted for histopathologic examination and bextra.

The abuse of Amphetamine Type Stimulants ATS ; has been a problem since the middle of the last century, though then confined to a handful of countries. Over the past ten years it has become a global problem, presenting an increasing, serious threat to the health, social and economic fabric of families, communities and nations. These issues have emerged more clearly as the problem has been studied over the last few years. In 1996, UNDCP began a comprehensive review of the ATS problem. Following an expert meeting in Vienna in February 1996, UNDCP published the findings of its study as Amphetamine-type Stimulants: A Global Review. The study was then examined, with a view to designing counter-measures, at a further expert meeting in Shanghai in November 1996. The recommendations of the Shanghai meeting were considered at the Commission of Narcotic Drugs, and refined into an action plan which became one of the measures adopted at the Special Session of the General Assembly on the World Drug Problem in June 1998. Recognizing the particular need for action in East and South-East Asia - the one sub-region of the world with a large ATS problem but a limited sub-regional framework to deal with it UNDCP initiated, with the support of the Government of Japan, a project to operationalize the ATS Action Plan in East and South-East Asia. The Conference on Amphetamine-Type Stimulants in East and South-East Asia was developed from these efforts. The Conference was attended by senior government officials concerned with ATS demand and regulatory issues in the countries or territories of Brunei Darussalam, Cambodia, China, Hong Kong SAR, India, Indonesia, Japan, Lao PDR, Malaysia, Mongolia, Myanmar, Philippines, Republic of Korea, Singapore, Thailand, and Vietnam. The purpose of the Conference was to keep up the momentum and raise awareness of ATS emerging as the key global drug problem in the 21st century, and to facilitate consensus on decisive and coordinated counter-actions by Governments in the East and South-East Asia region as well as from Governments in other regions of the world. Expected outcomes of the Conference included: Further raising of awareness on problems and issues related to a ; regulation and control of ATS and their precursors in East and South-East Asian countries, and b ; demand reduction and prevention; Exchange and sharing of information and good practices within and beyond the region; Identification of practical measures for national, sub regional and regional implementation of the ATS Action Plan; Information provided at the conference focused upon emerging regional and national issues in the context of reducing illicit demand as well as regulating ATS and their precursors. Illicit ATS abuse issues included limited supportive and collaborative relationships for the development and delivery of integrated drug control strategies, and lack of participation of risk groups, non-government organizations and community based groups in programme development. Information issues included the lack of reliable data on patterns of abuse and responses, and limitations in evaluation and programme monitoring methods and outcomes. Programme limitations were identified in the area of primary prevention related to mass and danazol.
