METHOD The population of our multi-centre study involved all the patients taking sildenafil for a period of at least six months from the three hospitals Changi General Hospital, National University Hospital and Tan Tock Seng Hospital. All the patients were aged 21 years or older. All of them have ED for a duration of at least six months. Those with DM have a disease duration of at least one year. The diagnosis of ED is based on the patient's medical history, physical examination, standard laboratory testing including hormonal profiles ; and other diagnostic procedures e.g. intracavernosal PGE 1 injections, duplex scans, cavernosometry and cavernosography ; . Exclusion criteria included the following: penile anatomical deformities that significantly impair erection; a primary diagnosis of sexual disorder other than ED; a major psychiatric disorder that was not well controlled with treatment; spinal cord injury; a history of major haematological, renal, or hepatic abnormalities; stroke or myocardial infarction within the previous six months; active peptic ulcer; hypotension; active proliferative diabetic retinopathy; or regular treatment with nitrates. The patients were given sildenafil at doses ranging from 12.5 mg to 100 mg. A higher dose of sildenafil was given if the initial dose was ineffective and if they were able to tolerate it. The patients were followed up after six months regarding their assessment of the efficacy of sildenafil on ED. Success in penetration and increased sustenance of erection were taken as target outcomes. The patients were given a global efficacy questionnaire GEQ ; , that is whether they thought that their erection has improved, pertaining to the above points. Although various scoring systems have been devised for erectile dysfunction, like the IIEF scores, most of our investigating doctors do not use such scores in their notes, this being one of the limitation of a retrospective study. The statistical analysis was performed with the aid of SPSS version 9.0.

HEMODYNAMIC EFFECTS: MYOCARDIAL OXYGEN SUPPLY The heart is almost exclusively dependent on aerobic metabolism and an adequate supply of oxygen is critical to sustained cardiac activity. Myocardial oxygen supply is a function of both oxygen delivery and oxygen extraction from the blood. Since oxygen extraction from coronary blood is near maximal at rest, there is little reserve to meet increased demand due to increased cardiac activity. Thus, the most important determinant of myocardial oxygen supply is total coronary blood flow. The nitrovasodilators have several effects on the coronary circulation, including dilation of large and intermediate-size coronary arteries, increased collateral flow, and redistribution of flow to ischemic regions of the heart. Accordingly, these beneficial effects are primarily responsible for the ability of nitrovasodilators to improve myocardial oxygen supply. ADVERSE EFFECTS AND PRECAUTIONS The most frequently observed adverse effects of the nitrates are a direct result of the vasodilation produced by these drugs 7 ; . Headache, due to dilation of cranial blood vessels, is common and may be severe. Symptoms of postural hypotension e.g. dizziness ; may also be encountered. Paradoxically, nitrates may increase myocardial oxygen demand in some patients by causing reflex tachycardia. A potentially dangerous drug interaction may occur with sildenafil, a phosphodiesterase inhibitor that increases intracellular cGMP accumulation. Concurrent use of nitrates and sildenqfil may cause a sudden and dramatic drop in blood pressure, prompting the FDA to issue a warning about prescribing sildenafil to patients being treated with nitrovasodilators. Although rarely used today as a therapeutic agent, amyl nitrite has gained popularity as a recreational drug. Inhalation of amyl nitrite, as well as isobutyl nitrite, purportedly enhances sexual pleasure and produces euphoria. Abuse of these products may cause severe cardiovascular toxicity. NITRATE TOLERANCE Continuous or repeated exposure to high doses of organic nitrates rapidly leads to a reduction in the hemodynamic and antianginal effects of these drugs 8 ; . This phenomenon, known as nitrate tolerance, may occur within 24-48 hours following exposure and represents a major limitation to the therapeutic use of organic nitrates. Tolerance is not usually observed with the occasional use of sublingual nitroglycerin, but is most often associated with the use of oral nitrates, transdermal patches, and continuous intravenous administration of nitroglycerin. The cause of nitrate tolerance is unclear but it is likely to be multifactorial. Several mechanisms have been proposed including: i ; decreased activation or desensitization of the NO cGMP signaling pathway including reduced bioconversion of nitrates to NO ii ; expansion of plasma volume; iii ; increased release and or sensitivity to endogenous vasoconstrictors, such as endothelin, angiotensin II, and catecholamines; and iv ; increased production of superoxide anions, which destroy NO. The most widely accepted method for preventing nitrate tolerance is to provide a period of low nitrate exposure during each day. For example, a common dosing strategy for transdermal nitroglycerin is to apply the patches for 12 hours and remove them for 12 hours each day. One concern with this approach is that the patient is unprotected during the nitrate-free interval and may require the use of additional antianginal drugs to prevent an acute attack and topamax.

A central mechanistic feature of both sickle cell disease and thalassemia is hemolytic anemia. Historically, this complication has been considered to increase only the risk for cholelithiasis and symptomatic anemia; however, more recent data suggest that intravascular hemolysis may produce a state of endothelial dysfunction and resistance to nitric oxide. This ultimately leads to smooth muscle and intimal hyperplasia, proliferative vasculopathy, and increased risk of thrombosis. In this context, it is now increasingly apparent that both diseases, as well as other hereditary and acquired hemolytic anemias, are associated with a high incidence and prevalence of pulmonary hypertension. Indeed, pulmonary hypertension is rapidly emerging as a major cause of early mortality in sickle cell disease and likely thalassemia ; . Although in a small subset of these patients cor pulmonale has been recognized for many decades, cases of mild or moderate pulmonary hypertension in most patients present few or no symptoms, yet still pose a very significant risk of early death. Agents to treat pulmonary hypertension in this population are promising in pilot studies, with larger-scale clinical trials needed to help guide clinical practice. In patients with pulmonary hypertension due to sickle cell disease or thalassemia, the greatest experience has been gained with sildenafil, and a large clinical trial is anticipated. The high prevalence of pulmonary hypertension in patients with thalassemia indicates that red cell sickling is unlikely to be the principal cause of pulmonary hypertension in patients with sickle cell disease. In these and other hemolytic disorders with a large intravascular component, other vascular complications such as priapism and cutaneous leg ulceration are emerging. Further, strong and consistent evidence is arising that intravascular hemolysis unleashes red cell contents into blood plasma, where they behave as chronic toxins that rob blood vessels of nitric oxide, a critical component in maintaining vascular health. In this issue, we review the current literature on the etiology, diagnosis, and management of pulmonary hypertension in patients with hemolytic disorders.

Three months after starting a diabetic diet, his OGTT was within normal limits fasting glucose, 102 mg dL; 2-hour glucose, 123 mg dL ; , and his neurologic deficits had stabilized. The initial presentation was consistent with mononeuropathy multiplex, including partial third nerve palsy. He went on to develop a painful polyneuropathy. He was ultimately found to have impaired glucose tolerance, perhaps exacerbated by steroids. Both mononeuropathy multiplex and albuminocytologic dissociation are seen with diabetes, and in retrospect, impaired glucose metabolism may have been the cause of his symptoms all along.
