Of fixed combination products. Increased imbalance of the National Health Insurance budget may necessitate the development of fixed combination antihypertensive drugs to curb health care costs. The Japanese National Health Insurance system, which aims at universal coverage and equal opportunity, is suffering from an increasing negative balance. Restricted payment of medical costs to hospitals by the insurers is included in the revision. IMPLICATIONS FOR FIXED COMBINATION ANTIHYPERTENSIVE PRODUCTS The advantages of combining antihypertensive drugs are: fewer side effects, achieved by reducing the dose of individual drugs; enhancement of the antihypertensive effect by promoting favorable drug interactions; improvement of patients' adherence to treatment; and improved cost-effectiveness. At present, International Conference on Harmonization ICH ; guidelines, designed to harmonize the evaluation and development of new antihypertensive drugs in the United States, Europe, and Japan, are being prepared.

Morphine is a strong opioid analgesic drug for oral administration for pain relief. It is particularly useful for treating severe continuous pain of visceral or soft tissue origins. Consultation paper on extending prescribing powers for nurses and pharmacists A consultation paper on "supplementary" prescribing by nurses and pharmacists has been published. Under this proposed scheme, supplementary prescribers will be able to prescribe from a more extensive range of medicines for a broad range of medical conditions. Prescribing will take place under the terms of a clinical management plan drawn up, with the patients agreement, in consultation with an independent prescriber, who will be a doctor. It is hoped that the proposals will help nurses and pharmacists to help more patients and treat more conditions, such as asthma, diabetes, hypertension and arthritis. The public consultation will close in July and it is envisaged that nurses will take up prescribing training by the end of 2002 and to be undertaking supplementary prescribing by early 2003, with pharmacists following behind. Altered taste or smell function can be very debilitating for patients, especially the elderly. It is now apparent that many modern antihypertensive drugs according to the Physician's Desk Reference, or PDR, 36% ; produce changes in chemosensory perception. Included are losses or distortions of taste qualities and the induction of phantom dysgeusias and dysosmias. Such sensory aberrations lead not only to a diminished quality of life, but also to decreased food intake, loss of appetite, weight decrement and depression. Some categories of antihypertensives, such as ACE inhibitors and calcium channel blockers, are particularly involved in alterations of taste and smell perception. It is helpful to know which particular drugs report relatively high incidences of these adverse effects, so that a drug with a similar therapeutic effect, but lacking the harmful side-effects, can be substituted. We examine these issues in detail, including an assessment of the prevalence of chemosensory side-effects in relation to common antihypertensive drugs, the physiological mechanisms likely involved and the.

Apomorphine, a dopamine receptor agonist, causes yawning in rats. It has been suggested that the analysis of yawning behavior provides an index of dopamine autoreceptor function. Dopamine turnover in the substantia nigra of diabetic rats has been shown to be decreased following administration of amphetamine or apomorphine 17, 21 ; . Yawning behavior after 4 weeks of streptozotocin STZ ; -induced diabetes in Wistar rats was significantly lowered when compared with their age-matched normal controls. Yawning behavior was not further diminished after an 8-week duration of diabetes mellitus; however, a significant recovery in yawning was seen by 20 weeks of diabetes. Yawning in rats after 20 weeks of STZ-induced diabetes mellitus is not significantly different from that seen in normal control rats. The results suggest that in STZ-induced diabetes of only 4 weeks duration a measurable change in the substrate for yawning has occurred. Heaton, J. P. and S. J. Varrin 1994 ; . The dependence of erectile behavior on androgen functioning is well established. Castration produces loss of both libido and potency in man and animals. The present study, using an animal model for potency, demonstrates the dependence of centrally induced erectile behavior on an intact androgen milieu. Castrated rats failed to produce an erection in response to apomorphine, an agent shown to produce erection in nearly all normal rats. Administration of exogenous testosterone propionate in dosages exceeding 60 micrograms. kg. produced a significant increase in erectile behavior. Yawning, an essentially parallel phenomenon to the stimulation of the erectile response, was also decreased following castration and responded similarly to increasing amounts of exogenous testosterone, demonstrating the influence of androgen functioning on the central nervous system. It was concluded that testosterone is a necessary prerequisite for the maintenance of a centrally induced erectile and yawning response. In an animal model of penile erection, testosterone increases the number of erections in a dose-dependent manner in castrated rats. According to the progress note written on 7 5 06, the physician was made aware of staff's concern regarding the resident's potential for further treatment and orders were received to send the resident to the hospital's "gero-psych" gerontology-psychiatry ; unit for evaluation. Nursing progress notes dated 7 5 06 describe the resident as dull, sitting in the wheelchair with arms limp and head down. The nursing progress note documented the resident stated "I feel dead inside, not sure how it can be fixed". A nursing progress note dated 7 11 06 revealed the resident was asked a minimum of six times if he wanted to go to the hospital and he nodded "yes" and said "if that's what it takes". Review of the facility's Policy for Comfort Care undated ; revealed a policy that the resident and family, along with the physician, will decide on the ultimate goal and treatment modalities. The policy documented the facility will support and maintain the patient's autonomy as much as possible.
