As a clinical trial endpoint, PSA remains problematic in some settings. In patients with biochemical failure only, using PSA failure as a parameter of response remains unproven; however, in the adjuvant setting, I think most of us who take care of these patients would clearly accept time to PSA failure as an endpoint in patients undergoing radical prostatectomy -- albeit not the only endpoint. Of course, reasonable assurances must be made to ensure that the PSA failures are real and not simply low levels of detectable PSA in patients who are not destined to progress. That's the optimal use of PSA in how we manage patients today, and it would be problem.
However, there are many challenges to offering effective suicide prevention services. Perhaps most important among these challenges is the lack of objective empirical evidence on what constitutes effective prevention. Simply put, the literature lacks sufficient evidence for managers and policy makers to build effective programs if they look only to empirical work. The reasons for this state of affairs is well known. As Charlton 1992 ; succinctly stated " Suicide rates are clearly influenced by factors outside health service control such as access to an effective means of committing suicide, divorce rates and employment levels. Suicide levels may therefore reflect changes in these factors rather than the success of any health service intervention" p. 10 ; . Furthermore, a number of problems consistently appear in suicide prevention research and evaluation. These include: 1. Historical inconsistency of a formal coordinating mechanism among regions.
