Study a decade, it will be a long time before we can answer the question about adjuvant chemotherapy in the treatment of prostate cancer. As a clinical trial endpoint, PSA remains problematic in some settings. In patients with biochemical failure only, using PSA failure as a parameter of response remains unproven; however, in the adjuvant setting, I think most of us who take care of these patients would clearly accept time to PSA failure as an endpoint in patients undergoing radical prostatectomy -- albeit not the only endpoint. Of course, reasonable assurances must be made to ensure that the PSA failures are real and not simply low levels of detectable PSA in patients who are not destined to progress. That's the optimal use of PSA in how we manage patients today, and it would be problem.
However, there are many challenges to offering effective suicide prevention services. Perhaps most important among these challenges is the lack of objective empirical evidence on what constitutes effective prevention. Simply put, the literature lacks sufficient evidence for managers and policy makers to build effective programs if they look only to empirical work. The reasons for this state of affairs is well known. As Charlton 1992 ; succinctly stated " Suicide rates are clearly influenced by factors outside health service control such as access to an effective means of committing suicide, divorce rates and employment levels. Suicide levels may therefore reflect changes in these factors rather than the success of any health service intervention" p. 10 ; . Furthermore, a number of problems consistently appear in suicide prevention research and evaluation. These include 1 ; problems of defining suicide, 2 ; deciding the appropriate source of study populations, 3 ; deciding the appropriate source of comparison groups, 4 ; retention in longitudinal research and 5 ; the low base rate of this phenomenon" Frederico & Davis, 1996 ; . These are fundamental research issues that profoundly affect the collection of reliable and valid information such as reporting of suicide incidence. It is widely recognized that even the World Health Organization statistics do not present a complete picture of the magnitude of the suicide phenomenon due to underreporting Diekstra, 1996 ; . Consequently it is not surprising that the literature consistently calls for good quality research utilizing operationalized definitions and adhering to the highest research standards Devlin, 2001; Leenaars, 2001; Middlebrook et al, 2001; Diekstra, 1996, Frederico, & Davis, 1996 ; . Many also acknowledge that it may be inappropriate, and perhaps impossible to apply classic positivist research standards such as those used in randomized clinical trials of drug interventions, to circumstances relevant to suicide prevention Gunnell & Frankel, 1994; Charlton et al, 1992; Jacobs, 1988 ; . Therefore, those who have responsibility for service delivery are in a quandary: they must make program and service delivery decisions that may seriously impact the lives of many individuals and they must do so without the benefit of conclusive empirical evidence. Like many jurisdictions, Alberta is attempting to prevent suicides among its population. Although Alberta has a reputation of being a leader in Suicide Prevention Leenaars et al, 1998; Pedersen 2001 ; , it has a number of additional challenges to overcome when considering how best to address prevention of suicide within the province. These include: 1. Historical inconsistency of a formal coordinating mechanism among regions. This has resulted in uneven service delivery despite the best efforts of Suicide Prevention Coordinators. 2. A geographically disperse population. 3. Aboriginal community needs that are complex and awareness that Aboriginal bands cannot be treated as a homogeneous group. 4. The uncertain future of suicide prevention service delivery across the province and diflucan.

We have shown selective effects of clonidihe on alerting and on both spatial and temporal aspects of attentional orienting performance in healthy human volunteers, whilst simultaneously measuring regional brain activity with fMRI to help elucidate the cognitive and neuroanatomical processes underlying these effects. Clonidine-induced deficits in the alerting effect and temporal orienting were accompanied by attenuation of taskassociated activity in distinct cortical regions. We suggest that this neuroanatomical distinction ref lects differential functional effects of the drug. Therefore, NA modulation of the alerting effect is unlikely to be simply the result of an underlying effect on temporal orienting. However, clonidine does impair temporal orienting performance and attenuates task-associated activity in areas previously implicated in orientation of attention to specific time inter vals. We have also shown that attenuation of right superior parietal cortex activity underlies the NA modulation of spatial attentional orienting. This suggests that the effects of the drug on this task are due to processes related to orienting to one or other side of space, rather than non-specific sensori-motor or arousal processes and dilantin and clonidine.

However, the method has never before been tested on bulky plant parts used as food. In our experiment, broccoli heads were stored in the cold for 12 days with various light treatments 6h per day 24 h darkness control ; . By using a reference fluorescence signal excited at 685 nm the non-destructive method worked well for the purpose. The absorption of chlorophyll fluorescence excited at wavelengths 382 nm or 530 nm decreased during storage irrespective of light treatment. The results indicate a breakdown of both UV-A-absorbing flavonoids and possibly also anthocyanins and diovan.
